and/or interviews for publicity purposes and to post such items on the Church's Website, we need your written permission and consent. To give your permission and consent, please answer yes in the box below. I give permission for my child to be photographed, videotaped, and/or interviewed by representatives from The Strong Tower for the purpose of publicizing church programs. I hereby authorize the use and reproduction by The Strong Tower or their authorized agent of any and all photographs, videotapes or other recording of any kind taken of my child, without compensation to me/my child. All photographs/video tapes or other recordings of any kind shall be the property, solely and completely, of the Church. I hereby waive any and all rights to inspect or approve the finished photographs, videotapes, and/or the soundtrack, script or printed material that may be used in connection with such photographs and/or videotapes.

This document is a Waiver and Release of Liability, which participants or their guardians sign to acknowledge the risks involved in participating in a specific program, such as those hosted by The Strong Tower church. By signing this waiver, the participant or guardian assumes full responsibility for any accidents, injuries, or other mishaps that may occur during the program, including severe outcomes like permanent disability or death. It explicitly states that the participant waives the right to file any claims or lawsuits against The Strong Tower or its affiliates, covering a range of people and entities associated with the church. Furthermore, the waiver releases all mentioned parties from any liability for personal injuries or property damage that may occur. This document is legally binding on the participant's or guardian's heirs and successors, ensuring that the waiver’s terms extend beyond the immediate signatory. If any part of the waiver is found invalid, the rest of the document remains effective.
